About the role

Key responsibilities & impact
  • Oversee Integrated Care Management (ICM) and Long-Term Services and Supports (LTSS) healthcare case management staff
  • Progress initiatives for high-performing health service teams
  • Collaborate with Health Services managers to ensure consistency and accuracy in clinical interventions
  • Complete tasks supporting financial, operational, and quality objectives
  • Serve as a key resource for healthcare members regarding governmental, state, local, and legal initiatives, including COBRA, EMTALA, and Medicare regulations
  • Implement and sustain Clinical Health Services policies, procedures, action plans, initiatives, and strategies
  • Provide application expertise to Health Services account managers to support client retention and resource utilization
  • Partner with patient resources, divisional management, Clinical Education, and other departments to evaluate performance initiatives and recommend training
  • Implement change management plans for new or enhanced initiatives
  • Facilitate regular staff meetings on policies, procedures, and expectations

Requirements

What you’ll need
  • 5+ years clinical work experience
  • Active and unrestricted applicable license/certification in the state of residence: Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Clinical Professional Counselor (LCPC)
  • Associate's Degree (REQUIRED)
  • Advanced problem solving and decision making skills
  • Advanced knowledge of medical terminology
  • Advanced collaboration and teamwork skills
  • Advanced growth mindset skills
  • Advanced skills in execution and delivery
  • Ability to evaluate relative costs and benefits of potential actions
  • Advanced digital literacy skills
  • Direct/hardwired internet connection to a modem/router within 7 feet of the computer
  • Minimum internet speed of 25 Mbps download and 3 Mbps upload
  • Quiet, secure, private designated home workspace compliant with CVS Health and HIPAA guidelines
  • Ability to use multiple monitors and navigate multiple applications
  • Ability to communicate through email, calendar invites, Teams messaging, and virtual meetings
  • Experience with Microsoft Office 365 or similar Google Workspace applications
  • Ability to access secure systems such as VPN and EHR portals and follow security practices
  • Ability to troubleshoot common technical issues independently
