About the role

Key responsibilities & impact

Collaborate with members, providers, and community organizations to address health care and social determinant needs
Facilitate appropriate healthcare outcomes for members
Assist with appointment scheduling
Identify and assist members with accessing benefits and education
Use care management tools and resources
Utilize critical thinking and judgment in the case management process
Travel throughout Western Illinois/Quad Cities for field-based work, up to 75% of the time
Communicate and collaborate with teams virtually
Navigate multiple systems and applications to streamline tasks and improve efficiency
Use digital communication channels, Teams, virtual meetings, VPN, and EHR portals
Resolve common technical issues and contact IT for unresolved issues
Learn new skills as the workplace environment evolves

Requirements

What you’ll need

Must reside in the state of Illinois
Must possess reliable transportation and be willing and able to travel up to 75% of the time from the home location in and around Western Illinois/Quad Cities
Minimum 3–5 years clinical practical experience
Associate's degree required
Active and unencumbered Registered Nurse license in the state of Illinois
Confidence working at home and independently using tools to collaborate virtually
Excellent analytical and problem-solving skills
Effective communication, organizational, and interpersonal skills
Ability to work independently
Effective computer skills, including navigating multiple systems and keyboarding
Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications
Direct/hardwired internet connection within 7 feet of the computer, with minimum 25 Mbps download and 3 Mbps upload speeds
Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines
Must work within the state and city where currently residing
Ability to use multiple monitors and applications
Ability to use email, calendar invites, Teams messaging, and virtual meetings
Experience with Microsoft Office 365 applications or similar Google Workspace tools
Ability to access secure systems such as VPN and EHR portals
Ability to manage strong passwords and recognize suspicious emails or links
Ability to troubleshoot common technical issues independently
Preferred: 2–3 years Care Management, discharge planning and/or home health care coordination experience
Preferred: Certified Case Manager
Bilingual preferred
