About the role

Key responsibilities & impact

Collaborate and inform the case management process using critical thinking and judgment
Facilitate appropriate healthcare outcomes for members
Assist with appointment scheduling
Identify and assist members with accessing benefits and education
Use care management tools and resources
Collaborate with members, providers, and community organizations
Address members’ health care and social determinant needs across the full continuum of care
Travel throughout the North/Northwest Suburbs of Chicago for field-based work while working from home when not traveling

Requirements

What you’ll need

Must reside in the state of Illinois
Must possess reliable transportation and be willing and able to travel up to 50-75% of the time from the home location in and around the North/Northwest Suburbs of Chicago
Minimum 3-5 years clinical practical experience
Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
Excellent analytical and problem-solving skills
Effective communication, organizational, and interpersonal skills
Ability to work independently
Effective computer skills, including navigating multiple systems and keyboarding
Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as proprietary applications
Associate's degree required; bachelor's preferred
Active and unencumbered Registered Nurse license in the state of Illinois
Direct/hardwired internet connection to a modem/router within 7 feet of the computer, with minimum 25 Mbps download and 3 Mbps upload speeds
WiFi and satellite internet are not permitted
Quiet, secure, private designated home virtual work location compliant with CVS Health and HIPAA guidelines
Must work within the state and city where currently living
Must provide own workspace furniture
Experience setting up and using multiple monitors and navigating multiple applications simultaneously
Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings
Experience with Microsoft Office 365 applications or similar Google Workspace applications
Ability to log in to secure systems such as VPN and EHR portals, secure devices, manage strong passwords, and recognize suspicious emails or links
Ability to resolve common technical issues independently
Preferred: 2-3 years Care Management, discharge planning and/or home health care coordination experience
Preferred: Certified Case Manager
Bilingual preferred
