About the role

Key responsibilities & impact

Utilize critical thinking and judgment to collaborate and inform the case management process
Facilitate appropriate healthcare outcomes for members
Assist with appointment scheduling
Identify and assist members with accessing benefits and education
Use care management tools and resources
Collaborate with members, providers, and community organizations across members’ health care and social determinant needs
Travel throughout DuPage, Kane, DeKalb, Kendall and surrounding West Suburbs Illinois areas
Work independently from a designated home workspace while collaborating virtually with teams
Navigate multiple systems, secure portals, and care management applications
Communicate through email, calendar invites, Teams messaging, and virtual meetings
Resolve common technical issues and contact IT for unresolved issues

Requirements

What you’ll need

Must reside in the state of Illinois
Must possess reliable transportation and be willing and able to travel up to 75% of the time from home location in and around West Suburbs IL, including DuPage, Kane, DeKalb, Kendall and surrounding areas
Minimum 3-5 years clinical practical experience
Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently
Effective computer skills including navigating multiple systems and keyboarding
Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications
Associate's degree required
Active and unencumbered Registered Nurse license in the state of Illinois
Direct/hardwired internet connection within 7 feet of the computer, with minimum 25 Mbps download and 3 Mbps upload speeds; WiFi and satellite internet are not permitted
Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines
Must work within the state and city where currently residing
Must provide workspace furniture
Ability to use multiple monitors and applications simultaneously
Ability to use email, calendar invites, Teams messaging, and virtual meetings
Experience with Microsoft Office 365 or similar Google Workspace applications
Ability to access secure systems such as VPN and EHR portals, follow computer security practices, and recognize suspicious emails or links
Ability to troubleshoot common technical issues independently
