About the role

Key responsibilities & impact

Collaborate and inform the case management process using critical thinking and judgment
Facilitate appropriate healthcare outcomes for members
Assist with appointment scheduling
Identify and assist members with accessing benefits
Provide education to members using care management tools and resources
Collaborate with members, providers, and community organizations
Address members’ health care and social determinant needs
Travel throughout Central Illinois, including Peoria, Tazewell, Knox, Stark, Fulton, Woodford, and Marshall areas
Work independently from a designated home office while using virtual collaboration tools

Requirements

What you’ll need

Must reside in the state of Illinois
Must possess reliable transportation and be willing and able to travel up to 75% of the time from home location in and around Central Illinois
Minimum 3-5 years clinical practical experience
Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently
Effective computer skills including navigating multiple systems and keyboarding
Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as proprietary applications
Associate's degree required
Active and unencumbered Registered Nurse license in the state of Illinois
Direct/hardwired internet connection to a modem/router within 7 feet of the computer
Minimum internet speed of 25 Mbps download and 3 Mbps upload
Quiet, secure, private designated home virtual work location compliant with CVS Health and HIPAA guidelines
Must work within the state and city where currently living
Must provide own workspace furniture
Microsoft Office 365, Teams, Outlook, Word, Excel, and PowerPoint, or similar Google Workspace experience
Ability to access secure systems such as VPN and EHR portals
Ability to manage strong passwords and recognize suspicious emails or links
Ability to troubleshoot common technical issues
Preferred: 2-3 years Care Management, discharge planning and/or home health care coordination experience
Preferred: Certified Case Manager
Bilingual preferred
