About the role

Key responsibilities & impact

Facilitate the case management process using critical thinking and judgment
Collaborate with members, providers, and community organizations
Facilitate appropriate healthcare outcomes for members
Assist with appointment scheduling
Identify and assist members with accessing benefits and education
Use care management tools and resources
Address members' health care and social determinant needs across the continuum of care
Travel throughout the West Suburbs of Illinois, including DuPage, Kane, DeKalb, Kendall and surrounding areas
Work independently from a designated home office while collaborating virtually with teams

Requirements

What you’ll need

Must reside in the state of Illinois
Must possess reliable transportation and be willing and able to travel up to 75% of the time from the home location in and around the West Suburbs of Illinois, including DuPage, Kane, DeKalb, Kendall and surrounding areas
Minimum 3–5 years clinical practical experience
Confidence working at home and independently, using tools to collaborate and connect with teams virtually
Excellent analytical and problem-solving skills
Effective communication, organizational, and interpersonal skills
Ability to work independently
Effective computer skills, including navigating multiple systems and keyboarding
Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, PowerPoint, and proprietary applications
Active and unencumbered Registered Nurse license in the state of Illinois
Direct/hardwired internet connection to a modem/router within 7 feet of the computer, with minimum speeds of 25 Mbps download and 3 Mbps upload; WiFi and satellite internet are not permitted
Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines
Must work within the state and city where currently residing
Associate's degree required
Ability to use multiple monitors and applications simultaneously
Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings
Experience with Microsoft Office 365 applications or similar tools such as Google Workspace
Ability to access secure systems such as VPN and EHR portals, follow computer security practices, manage strong passwords, and recognize suspicious emails or links
Ability to resolve common technical issues independently and contact IT for unresolved issues
