About the role

Key responsibilities & impact

Collaborate and inform the case management process using critical thinking and judgment
Facilitate appropriate healthcare outcomes for members
Assist with appointment scheduling
Identify and assist members with accessing benefits and education
Use care management tools and resources
Serve Medicare and Medicaid dual-eligible members with complex health and social challenges
Collaborate with members, providers, and community organizations
Address members’ health care and social determinant needs
Travel throughout East Central Illinois, including Champaign, Vermilion, Ford, Piatt, Douglas, Coles, and Edgar counties
Work from a designated home virtual work location and collaborate with teams virtually

Requirements

What you’ll need

Must reside in the state of Illinois
Must possess reliable transportation and be willing and able to travel up to 50-75% of the time from home location in and around East Central Illinois
Minimum 3-5 years clinical practical experience
Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently
Effective computer skills including navigating multiple systems and keyboarding
Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as proprietary applications
Associate's degree required
Active and unencumbered Registered Nurse license in the state of Illinois
Direct/hardwired internet connection with minimum 25 Mbps download and 3 Mbps upload; WiFi and satellite internet are not permitted
Quiet, secure and private designated home virtual work location compliant with CVS Health and HIPAA guidelines
Must work within the state and city where currently living
Must provide own workspace furniture
Ability to use multiple monitors and navigate multiple applications
Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings
Experience with Microsoft Office 365 applications or similar Google Workspace
Ability to log in to secure systems such as VPN and EHR portals, maintain security practices, and recognize suspicious emails or links
Ability to resolve common technical issues independently
Openness to learning new skills
