About the role

Key responsibilities & impact

Utilize critical thinking and judgment to collaborate and inform the case management process
Facilitate appropriate healthcare outcomes for members
Assist with appointment scheduling
Identify and assist members with accessing benefits and education
Use care management tools and resources
Collaborate with members, providers, and community organizations
Address members’ health care and social determinant needs
Travel up to 75% throughout Metro East/Southern Illinois and surrounding counties
Work independently from home while connecting with teams virtually

Requirements

What you’ll need

Must reside in the state of Illinois
Must possess reliable transportation and be willing and able to travel up to 75% of the time from home location in and around Metro East/Southern Illinois
Minimum 3-5 years clinical practical experience
Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently
Effective computer skills including navigating multiple systems and keyboarding
Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications
Associate's degree required
Active and unencumbered Registered Nurse license in the state of Illinois
Direct/hardwired internet connection with minimum 25 Mbps download and 3 Mbps upload; WiFi and satellite internet are not permitted
Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines
Must work within the state and city where currently living
Must provide own workspace furniture
Ability to use multiple monitors and applications simultaneously
Ability to use digital communication tools and virtual meetings
Experience with Microsoft Office 365 or similar Google Workspace
Ability to access secure systems such as VPN and EHR portals and follow security practices
Ability to resolve common technical issues independently
Preferred: 2-3 years Care Management, discharge planning and/or home health care coordination experience
Preferred: Certified Case Manager
Bilingual preferred
