About the role
Key responsibilities & impact- Facilitate appropriate healthcare outcomes for waiver/LTSS members
- Provide care coordination, support and education for members using care management tools and resources
- Manage a full caseload of waiver members
- Meet with members face to face in the field
- Complete in-person quarterly visits with members
- Travel up to 75% of the time to meet with members face to face
- Meet contractual requirements
Requirements
What you’ll need- 2 years’ experience in behavioral health, social services or appropriate related field equivalent to program focus
- Must reside in Richmond Virginia or Surround Areas
- Must possess reliable transportation
- Must be willing and able to travel up to 70% of the time
- Experience in Microsoft Office Suite
- Bachelor’s degree or non-licensed master level clinician required, with either degree being in behavioral health or human services
- Case management and discharge planning experience preferred
- Managed Care experience preferred
- LTSS experience preferred
- Certification in LMHP, RN/LPN, QMHP, LMSW, LBSW, MSW or BSW preferred
