About the role

Key responsibilities & impact
  • Work with contracted providers in Phoenix, Arizona
  • Conduct comprehensive evaluations of referred members’ needs and eligibility
  • Recommend approaches to case resolution and meeting member needs
  • Identify high-risk factors and service needs and make appropriate referrals
  • Coordinate and implement assigned care plan activities and monitor progress
  • Present cases at case conferences for multidisciplinary review
  • Identify and escalate quality-of-care issues
  • Negotiate appropriate options and services for members
  • Use motivational interviewing to promote member engagement and healthy behavior changes
  • Provide coaching, information, and support for independent medical and healthy lifestyle choices
  • Help members participate in healthcare decision-making with providers
  • Apply case management processes in compliance with regulatory, accreditation, and company guidelines

Requirements

What you’ll need
  • 2+ years of experience in behavioral health
  • Must reside in Maricopa County, Arizona
  • Ability to travel to providers’ offices up to 25% of the time within Phoenix, Arizona
  • Bachelor's degree
  • Case management and discharge planning experience preferred
  • Managed Care experience preferred
  • Non-licensed Master’s level clinician preferred
