About the role
Key responsibilities & impact- Assess, plan, implement, and coordinate case management activities telephonically and/or face to face
- Evaluate members’ medical needs to facilitate overall wellness
- Develop proactive courses of action to address issues and improve short- and long-term outcomes
- Review clinical tools, information, data, prior claims, benefit eligibility, and member needs
- Facilitate transitions to Aetna programs and plans
- Apply clinical judgment to reduce risk factors and barriers and address complex health and social indicators
- Assess co-morbid and multiple diagnoses affecting functionality
- Assess work capacity and related restrictions or limitations
- Determine the need for referrals to clinical resources
- Consult with supervisors and others to overcome barriers and present cases at multidisciplinary case conferences
- Use case management processes in compliance with regulatory and company policies and procedures
- Conduct interviews to engage members and determine health status and needs
Requirements
What you’ll need- Candidate must reside in southern Illinois and surrounding counties
- Minimum of 2 years of experience in case management, behavioral health, social services, or related field
- Willing and able to travel using your own vehicle 50-75% of their time to meet with members face to face in surrounding areas
- Valid Drivers License, preferably in IL
- Bachelor's Degree or non-licensed master level clinician required, preferably with a degree either being in behavioral health or human services
- Long Term Care Experience preferred
- Case Management or Discharge Planning experience preferred
- Managed care experience preferred
- Microsoft Office (Excel) experience preferred
