About the role
Key responsibilities & impact- Provide comprehensive healthcare management services
- Facilitate appropriate healthcare treatment
- Manage healthcare costs and improve healthcare program and operational efficiency involving clinical issues
- Determine medical necessity and appropriateness
- Facilitate optimal outcomes
- Identify and follow through with continuous quality and compliance opportunities
- Identify aberrances and initiate corrective action when applicable
- Educate and empower customers to ensure compliance and satisfaction and promote patient advocacy
- Optimize total costs
- Implement and evaluate policy based on usage and program directives
- Educate and empower colleagues at all levels to enable decision-making at the most appropriate level
- Travel 50–75% to meet members face to face in South Cook County and surrounding areas
Requirements
What you’ll need- Minimum 2 years of experience in behavioral health, social services or human services field
- Minimum 2 years of case management experience
- Must reside in South Cook County or Will County, Illinois, or surrounding areas
- Must live particularly in ZIP codes 60461, 60443, 60471, or 60466
- Willing and able to travel using your own vehicle 50–75% of the time to meet members face to face
- Reliable transportation required
- Valid Illinois Driver’s License
- Bachelor's degree or non-licensed master level clinician required, in behavioral health, social services or human services
- Case management and discharge planning experience preferred
- Managed care experience preferred
- Microsoft Office experience preferred, specifically Excel
