About the role

Key responsibilities & impact
  • Assess members through regular in-person or telephonic contact
  • Plan, implement, and coordinate case management activities
  • Evaluate members’ medical, psychosocial, physical, behavioral, and functional needs
  • Develop proactive courses of action to improve short- and long-term health outcomes and wellness
  • Use clinical assessment tools and data review to evaluate needs and benefit-plan eligibility
  • Apply clinical judgment to reduce risk factors and address complex health and social barriers
  • Integrate assessment data from care partners and address comorbidities and multiple diagnoses
  • Create, monitor, and revise comprehensive member care plans
  • Review prior claims for potential impact on case management and eligibility
  • Assess functional capacity and restrictions or limitations
  • Refer members to additional clinical resources when needed
  • Consult with supervisors and care partners to overcome barriers
  • Present cases at multidisciplinary case conferences
  • Follow regulatory and company case-management policies and procedures
  • Use motivational interviewing to engage members and assess health needs
  • Support members in participating knowledgeably in healthcare decision-making
  • Work in a field-based environment with productivity and quality expectations
  • Travel routinely throughout the Two Rivers Region and attend Louisville office meetings and training

Requirements

What you’ll need
  • 3+ years’ clinical practice experience including patient care coordination and health assessments
  • RN with current unrestricted state licensure in KY
  • 2+ years’ experience using personal computers, keyboard navigation, multiple systems and applications, and MS Office Suite applications including Teams, Outlook, Word, and Excel
  • Reliable transportation and willingness and ability to travel in-state up to 80–90% of the time
  • Valid KY driver’s license and proof of vehicle insurance
  • Flexibility to work beyond core business hours of Monday–Friday, 8am–5pm CST
  • Must reside in one of the specified Two Rivers Region counties in Kentucky
  • Minimum of an Associates degree in Nursing or Diploma RN
  • BSN preferred
  • Preferred knowledge or experience with Kentucky Foster Care and juvenile justice systems, Adoption Assistance, Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention services, and evidence-based practices
  • Case Management in an integrated model
  • Discharge Planning experience
  • Motivational Interviewing skills
  • Managed Care/Utilization Review experience
  • Knowledge of age-appropriate developmental milestones
  • Knowledge of complex medical conditions and treatment plans
  • Effective verbal and written communication skills
  • Bilingual ability preferred
