About the role
Key responsibilities & impact- Conduct comprehensive telephonic and occasional in-person assessments
- Develop and implement personalized care plans
- Identify and address clinical, social, and behavioral health needs
- Collaborate with multidisciplinary teams to support holistic care
- Guide members through benefits, resources, and care transitions
- Use clinical tools and data to reduce risks and improve outcomes
- Navigate multiple systems and applications to streamline tasks
- Communicate via email, calendar invites, Teams messaging, and virtual meetings
- Maintain secure access to systems and follow workplace security procedures
- Troubleshoot common technical issues and contact IT when needed
Requirements
What you’ll need- Active, unrestricted multi-state RN license in state of residence
- 3+ years of clinical RN experience
- 1+ year experience using multiple systems (e.g., Excel, Outlook, web-based tools)
- Strong assessment, communication, and critical thinking skills
- Willingness to obtain additional state licenses
- Ability to obtain a direct/hardwired internet connection to a modem/router within 7 feet of the computer
- Minimum internet speed of 25 Mbps download and 3 Mbps upload
- Quiet, secure, private designated home workspace compliant with CVS Health and HIPAA guidelines
- Ability to work within the state and city where currently living
- Ability to set up and use multiple monitors and navigate multiple applications
- Experience with Microsoft Office 365 applications or similar Google Workspace tools
- Ability to access secure systems such as VPN and EHR portals
- Ability to follow computer security practices and recognize suspicious emails or links
- Ability to troubleshoot common technical issues
- Preferred: Case management experience
- Preferred: Bilingual Spanish/English
- BSN preferred
