About the role

Key responsibilities & impact

Conduct comprehensive telephonic and face-to-face assessments of members’ medical, behavioral, and social needs
Develop and implement personalized short- and long-term care plans
Coordinate care across providers, programs, and services
Use clinical expertise and data insights to identify risks, gaps in care, and intervention opportunities
Support members with complex or multiple conditions using a holistic, whole-person approach
Facilitate referrals to clinical and community resources
Participate in case conferences and collaborate with multidisciplinary teams
Review claims history and benefit eligibility to inform care planning
Engage members through interviewing to build trust and encourage participation in their care
Ensure case management activities comply with regulatory and organizational guidelines
Blend in-person field visits with telephonic outreach to deliver coordinated, patient-centered care

Requirements

What you’ll need

Active, unrestricted Registered Nurse (RN) license in Virginia
Residence in Tidewater, Newport News, Hampton and Williamsburg Virginia areas
Reliable transportation
Willingness and ability to travel up to 75% of the time
3+ years of clinical nursing experience, such as medical-surgical, acute care, or behavioral health
Strong communication, organization, and critical-thinking skills
Comfort working with technology, including multiple systems and documentation platforms
Associate Degree in Nursing (required)
Case management experience, particularly within an integrated care model (preferred)
Bilingual English/Spanish (preferred)
Experience working with youth with mental health diagnoses (preferred)
Ability to manage multiple priorities effectively in a fast-paced, dynamic environment (preferred)
