Key responsibilities & impact

Facilitate appropriate healthcare outcomes for members
Assist with appointment scheduling
Identify and assist members with accessing benefits and education

Collaborate with members, providers, and community organizations
Address members’ healthcare and social determinant needs
Travel from the home location in and around Kane and DuPage Counties and surrounding areas, IL

Requirements

What you’ll need

Must reside in the state of Illinois
Must possess reliable transportation and be willing and able to travel up to 50-75% of the time from home location in and around Kane and DuPage Counties and surrounding areas, IL
Minimum 3-5 years clinical practical experience
Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently
Effective computer skills including navigating multiple systems and keyboarding
Demonstrates proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as some special proprietary applications
Associate's degree required
Active and unencumbered Registered Nurse license in the state of Illinois
Direct/hardwired internet connection with minimum 25 Mbps download and 3 Mbps upload; WiFi and satellite internet are not permitted
Quiet, secure and private designated home virtual work location compliant with CVS Health and HIPAA guidelines
Must work within the state and city where currently living
Must provide own workspace furniture
Must obtain required internet speeds and adhere to Work From Home requirements if hired
Comfortable setting up and using multiple monitors and navigating multiple applications simultaneously
Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings
Experience with Microsoft Office 365 applications or similar Google Workspace
Ability to log in to secure systems such as VPN and EHR portal, maintain computer security, manage strong passwords, and recognize suspicious emails or links
Ability to resolve common technical issues independently and contact IT for unresolved issues
Openness to learning new skills as the workplace environment evolves
