About the role
Key responsibilities & impact- Assess, plan, and coordinate comprehensive care for members with diverse medical and behavioral health needs
- Conduct in-depth, face-to-face assessments to identify member needs
- Develop and implement individualized care plans
- Coordinate medical, behavioral, and community-based services
- Monitor progress and adjust care strategies to improve outcomes
- Advocate for members to ensure access to high-quality, appropriate care
- Integrate services and address barriers to care
- Connect members with appropriate healthcare and community resources
- Build relationships with members through in-person visits
- Collaborate with interdisciplinary teams, providers, and support networks
- Work autonomously while coordinating proactive, member-centered care
Requirements
What you’ll need- Candidate must reside in Roanoke or surrounding regions in Virginia
- Reliable transportation
- Willingness and ability to travel up to 75% of the time
- Current unrestricted Virginia Registered Nurse (RN) state licensure required
- 3 years of clinical experience, preferably in Medical-Surgical Nursing, Behavioral Health, Long Term Care, or similar areas
- 1+ years of Managed Care experience
- Proficiency with computer skills, including navigating multiple systems and keyboarding
- Associate’s Degree or Bachelor’s in Nursing, with active Registered Nurse (RN) licensure required
- Bachelor’s Degree in Nursing preferred
- Case Management in an integrated model preferred
- 5 years clinical practice experience preferred
- Effective verbal and written communication skills
- Bilingual in English/Spanish preferred
- LTSS experience preferred
