About the role

Key responsibilities & impact

Conduct telephonic assessments to identify members’ medical, behavioral, and social needs
Develop and coordinate personalized care plans
Collaborate with healthcare providers and multidisciplinary teams to remove barriers to care
Use clinical judgment to address complex health conditions and improve outcomes
Support members through transitions of care and connect them with appropriate resources
Maintain compliance with regulatory and company case management standards
Work Monday-Friday, 9:00 AM-5:30 PM EST
Occasionally work evenings, weekends, and holidays
Undertake limited travel of up to 10%

Requirements

What you’ll need

Active, unrestricted RN license in North Carolina
Must reside in North Carolina
Minimum 2 years of RN clinical experience
Associate Degree in Nursing
Experience in any clinical setting considered, including ICU, NICU, Emergency Department, Med/Surg, Telemetry, OR, Home Health, Hospice, Long-Term Care, Infusion, and Orthopedics
Strong communication and member engagement skills
Direct/hardwired internet connection to a modem/router within 7 feet of the computer
Minimum internet speed of 25 Mbps download and 3 Mbps upload
WiFi and satellite internet are not permitted
Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines
Must work within the state and city where currently residing
Ability to use multiple monitors and navigate multiple applications simultaneously
Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings
Experience with Microsoft Office 365 or similar Google Workspace applications
Ability to log in to secure systems such as VPN and EHR portals
Ability to manage strong passwords and recognize suspicious emails or links
Ability to resolve common technical issues independently
Preferred: Certified Case Manager (CCM)
Preferred: BSN
Preferred: Case Management or Managed Care experience
Preferred: Discharge Planning experience
Preferred: Proficiency with Microsoft Office Suite
