About the role

Key responsibilities & impact

Coordinate all case management activities with members to evaluate medical needs
Facilitate the overall wellness of members
Develop strategies to address issues, outcomes, and opportunities to enhance members' overall wellness through integration
Instruct on programs and procedures in compliance with network management and clinical coverage policies
Travel within 125 miles round trip for 1 day in the field
Work a variable schedule, including 2 days weekly until 9pm local time
Achieve performance metrics and maintain compliant documentation

Requirements

What you’ll need

Must reside in Louisiana in one of the following parishes: Terrebonne, Lafourche, St. Charles, St. Johns, St. James, St. Mary, or Assumption
Current/Active Clinical Licensure (RN, LCSW, LPC, LMFT)
Case Management experience
Strong customer service orientation and problem solving
Excellent Communication/Telephonic skills
Excellent motivational interviewing skills/ability to built rapport and trust telephonically
Excellence in documentation and respecting compliance/regulatory standards
Highly organized, remain self-driven without direct supervision
Manage time effectively
Achieve performance metrics
Technology proficiency
Ability to travel within 125 miles round trip for 1 day in the field
Variable work schedule with ability to work 2 days weekly until 9pm local time
Bachelor's and/or Master's Degree listed under Education
Preferred: CCM Certification
Preferred: Previous experience in Managed Care Organization
