  • Use personal recovery experience to help the Aetna team coordinate member care
  • Share coping strategies, wellness tools, and resources with care managers and coordinators
  • Conduct field-based, face-to-face, and virtual member visits
  • Attend case rounds and help identify members who may benefit from Peer Support Services
  • Educate care management teams on peer support services and recovery goals
  • Collaborate with healthcare professionals, providers, members, and community agencies
  • Help members identify socioeconomic issues and develop person-centered plans and goals
  • Attend medical, behavioral health, or community appointments when appropriate
  • Advocate for members’ needs and individualized goals
  • Connect members with community-based services and supports
  • Research available community-based supports and services
  • Use open-ended questions to engage members in dialogue
  • Document non-clinical, person-centered member outreach notes
  • Manage schedules, competing projects, and demands independently
  • Maintain continuing education requirements for Louisiana Peer Support Specialists

Requirements

What you’ll need
  • Must reside in Louisiana, specifically in the southeastern portion of the state (Baton Rouge, Northshore, New Orleans)
  • Ability to travel 50% of the time to multiple office and home locations
  • Personal vehicle, reliable transportation, valid driver's license, and automobile insurance required
  • 1-2 years’ experience in a community/outpatient setting working with members in a mental health or substance use setting
  • 1-2 years’ experience providing care to socially and medically complex patients in varied non-traditional settings
  • 2-3 years of knowledge of formal and informal community agencies and resources
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services
  • Ability to educate health care providers to adjust interventions based on cultural, ethnic, or religious preferences
  • Experience using computers and strong keyboard navigation skills, including MS Office Suite, Word, Teams, and Excel
  • Ability to collaborate in a team and manage multiple priorities
  • Effective time management and sound judgment
  • Proven ability to identify solutions and execute tasks independently
  • Exceptional communication, organizational, and interpersonal skills with attention to detail
  • Ability to work effectively with diverse constituencies
  • Bilingual skills desired
  • Peer Support Specialist certification (RPSS) required per State of Louisiana guidelines
  • High School Diploma or GED
  • Preferred: experience serving Medicaid populations in urban or rural environments with familiarity with local formal and informal resource networks
  • Preferred skills: Mental Health First Aid, Trauma Informed Care, HIPPA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions
