About the role
Key responsibilities & impact- Conduct comprehensive, face-to-face assessments for members enrolled in MLTSS and D-SNP programs
- Develop and implement personalized care plans addressing physical, behavioral, and psychosocial needs
- Collaborate with members, families, providers, and interdisciplinary teams to coordinate care
- Evaluate and facilitate adult and pediatric medical daycare, personal care assistance, nursing facility support, and community-based and long-term services
- Advocate for members’ access to appropriate, high-quality care
- Authorize services and ensure efficient, cost-effective resource utilization
- Document care activities accurately in the electronic health record
- Participate in interdisciplinary meetings and drive positive outcomes
- Support and mentor new team members
- Conduct meaningful in-person community engagement across Bergen and surrounding counties
Requirements
What you’ll need- Licensed Clinical Social Worker (LCSW), or Licensed Social Worker (LSW)
- 2+ years of Behavioral Health experience/expertise required
- Reliable transportation
- Willing and able to travel up to 75% of the time
- Successful completion of the NJ Choice certification as a contingency of employment
- Exam score of 80% or higher for NJ Choice certification
- Successful completion of State training modules and field mentoring component
- Experience in case management or discharge planning preferred
- Managed care experience preferred
- Strong crisis intervention skills preferred
- Master’s Degree
- Independent license in NJ LSW or LCSW
