About the role

Key responsibilities & impact

Conduct comprehensive clinical assessments integrating medical, behavioral health, and psychosocial needs
Develop, implement, and monitor individualized care plans with members and providers
Coordinate care with medical and behavioral health professionals
Provide crisis intervention, timely referrals, and follow-up support
Use motivational interviewing and coaching to engage members and promote sustainable health behavior changes
Identify quality of care concerns and escalate appropriately
Maintain accurate, timely documentation in compliance with regulatory and accreditation standards

Requirements

What you’ll need

Active, unrestricted independent clinical license in the candidate’s state: LCSW, LSW, LISW, LCPC, LPC, LP, LMFT, or LMHC
3+ years of post-master’s direct clinical experience
3+ years of strong knowledge of mental health and substance use disorders
3+ years of experience with crisis intervention and motivational interviewing
3+ years of case management experience
Ability to multitask in a fast-paced, telephonic work environment
Strong technical skills
Master’s degree in social work or counseling required
