About the role

Key responsibilities & impact
  • Serve as a bridge between members/community and the health care system/providers through outreach and education
  • Help Aetna members attain and maintain better health outcomes
  • Improve relationships between members and healthcare providers
  • Help members and providers become more acculturated with each other
  • Work under the direction of a designated professional
  • Provide support care to socially and medically complex patients in homes and other non-traditional settings
  • Communicate medical information to healthcare professionals and care coordinators by telephone
  • Travel to multiple office locations and home locations
  • Work with members and the community in Champaign and surrounding counties
  • Work collaboratively with teams while managing multiple priorities
  • Work Monday through Friday from 8:00 a.m. to 5:00 p.m., with occasional weekend hours
  • Travel up to 70% of the time

Requirements

What you’ll need
  • Must be located in Champaign, IL or surrounding area
  • Must be willing to travel within the 90 mile radius from Champaign County, IL at least 70% of the time
  • Must possess reliable transportation
  • 1-3 years relevant experience, such as community outreach or home visits to members/patients
  • Experience serving Medicaid populations in urban or rural environments
  • Familiarity with local, formal, and informal resource networks
  • Flexibility to work in unknown working conditions, including varying patient-home environments
  • Ability to communicate medical information to health care professionals and care coordinators by telephone
  • Knowledge of formal and informal community agencies and resources
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services
  • Strong interpersonal and communication skills
  • Ability to work collaboratively, manage multiple priorities, use effective time management, and exercise sound judgment
  • Exceptional organizational and interpersonal skills with attention to detail
  • Ability to support socially and medically complex patients in varied non-traditional settings
  • Ability to influence medical care providers to adjust interventions based on members’ cultural, ethnic, or religious preferences
  • Community Health Worker Certification or equivalent training and experience
  • Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions preferred
  • Meet local state requirements such as first aid, CPR, and TB screening
  • Computer experience, including Excel and Electronic Health Records
  • High School Diploma or G.E.D.
