About the role
Key responsibilities & impact- Conduct home visits with Aetna members with chronic conditions
- Reinforce care-plan adherence and adapt tasks to members’ culture, language, and religion
- Educate healthcare professionals and providers about social determinants of health
- Build relationships with members, providers, nurse case managers, agencies, supervisors, and office staff
- Reduce cultural and socioeconomic barriers between clients and institutions
- Assist with care-plan implementation, member education, care-management strategies, and community linkages
- Help members identify socioeconomic issues and develop health/social management plans and goals
- Identify care-system gaps and help members access community services and eligible programs
- Schedule social-service appointments and assist with applications
- Support medication adherence, medication reviews, and refill acquisition
- Teach disease self-management, including nutrition and symptom tracking
- Accompany members to appointments as needed
- Maintain electronic health records, compile reports, complete progress notes and letters, and perform administrative duties
- Travel extensively to client homes, community locations, agencies, and outreach destinations
Requirements
What you’ll need- Must reside in Louisiana and within assigned Region 1 parishes: Orleans, Jefferson, St. Bernard, Plaquemines
- If not CHW certified at hire, must successfully complete a Certified Community Health Worker / Promotora certification program recognized by the State of Louisiana within 12 months of start date
- Ability to travel 60-75% of the time
- Must have access to reliable transportation
- 1-2 years’ experience providing clinical support services
- 1 year of community outreach experience and knowledge of Louisiana community resources
- 1 year of experience working with clients with clinical needs (physical and/or behavioral health)
- Proficiency with a personal computer, keyboard navigation, MS Office Suite, MS Teams, Outlook, Word, Excel, and electronic health record systems
- Flexibility to work outside core business hours Monday-Friday, 8am-5pm
- Ability to work two days weekly until 9pm local time
- High school diploma or G.E.D.
- Bilingual speaking skills in Spanish or French are a plus
