About the role

Key responsibilities & impact

Remove barriers related to Social Determinants of Health, access to care, and access to needed healthcare services
Serve as a bridge between members and the healthcare system through outreach and education
Conduct face-to-face visits within Sarasota and Manatee Counties
Help Aetna members attain and maintain better health outcomes
Improve relationships between members and healthcare providers
Support vulnerable members through an initiative addressing their needs

Requirements

What you’ll need

Candidate must reside in Sarasota or Manatee County, Florida
Willingness and ability to travel up to 50% of the time in Sarasota or Manatee County, Florida
2+ years experience providing community based services
Experience in community/outpatient setting as well as public health
High School Diploma/GED or equivalent experience (REQUIRED)
Comfortable setting up and using multiple monitors and navigating multiple applications simultaneously
Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings
Experience with Microsoft Office 365 applications or similar Google Workspace applications
Ability to log in to secure systems such as VPN and EHR portals
Ability to manage strong passwords and recognize suspicious emails or links
Ability to resolve common technical issues independently
Openness to learning new skills as the workplace environment evolves
