About the role
Key responsibilities & impact- Serve as a bridge between Aetna members and the healthcare system through outreach and education
- Conduct home visits with members with chronic conditions and reinforce care-plan adherence
- Adjust services to members’ culture, language, and religion
- Identify healthcare needs, improve access to care, and reduce unnecessary hospitalizations
- Educate healthcare professionals and providers about social determinants of health
- Build and maintain relationships with members, providers, nurse case managers, agencies, supervisors, and office staff
- Reduce cultural and socioeconomic barriers between clients and institutions
- Assist with care-plan implementation, member education, and care-management strategies
- Link members to health and social services and help identify management plans and goals
- Identify care-system gaps and help members use community services, schedule appointments, and complete program applications
- Support medication adherence by reviewing medication lists and assisting with refills
- Teach disease self-management, including nutrition and symptom tracking
- Accompany members to appointments as needed
- Maintain member records in electronic health records, compile reports, complete progress notes and letters, and perform administrative duties
- Travel extensively to client homes, community locations, agencies, and outreach destinations
- Work under the direction of a designated professional
Requirements
What you’ll need- Must reside in Louisiana and within assigned Region 7 parishes: Caddo, Bossier, Webster, Claiborne, Bienville, Red River, DeSoto, Sabine, Natchitoches
- If not CHW certified at hire, must successfully complete a Certified Community Health Worker / Promotora certification program recognized by the State of Louisiana within 12 months of start date
- Ability to travel 60–75% of the time
- Must have access to reliable transportation
- 1–2 years’ experience providing clinical support services
- 1 year of community outreach experience and knowledge of Louisiana community resources
- 1 year of experience working with clients with clinical needs (physical and/or behavioral health)
- Demonstrated proficiency with a personal computer, keyboard navigation, MS Office Suite, MS Teams, Outlook, Word, Excel, and electronic health record systems
- Flexibility to work outside core business hours Monday–Friday, 8am–5pm, to meet member needs
- Variable work schedule with ability to work 2 days weekly until 9pm local time
- High school diploma or G.E.D.
- Bilingual speaking skills are a plus: Spanish or French
- Preferred: community/outpatient setting experience; Mental Health First Aid; Trauma Informed Care; HIPAA; C.L.A.S. Standards; Motivational Interviewing; specialized medical terminology and practices training; Medicaid population experience; Community Health Worker Certification or equivalent training and experience
