About the role
Key responsibilities & impact- Serve as a bridge between Aetna members and the healthcare system through outreach and education
- Conduct home visits with members who have chronic conditions
- Reinforce care-plan adherence and adapt tasks to members’ culture, language, and religion
- Educate healthcare professionals and providers about social determinants of health affecting members
- Build and maintain relationships with members, providers, case managers, agency representatives, supervisors, and office staff
- Reduce cultural and socioeconomic barriers between clients and institutions
- Assist with care-plan implementation, member education, and care-management strategies
- Link members to health and social services and help identify socioeconomic issues affecting health
- Identify gaps in community care systems and assist members with community services, appointments, transportation vendors, and program applications
- Support medication adherence by reviewing medication lists and helping members obtain refills
- Teach disease self-management, including nutrition and symptom tracking/reporting
- Accompany members to appointments as needed
- Maintain member records in electronic health record systems and complete progress notes, letters, reports, and other documentation
- Travel extensively to client homes, community locations, agencies, and outreach destinations
Requirements
What you’ll need- Must reside in Louisiana and within assigned Region 9: St. Tammany, Livingston, Washington, or Tangipahoa
- Must be CHW certified at hire or successfully complete a Certified Community Health Worker/Promotora certification program recognized by the State of Louisiana within 12 months of start date
- Ability to travel 60–75% of the time
- Access to reliable transportation
- 3–5 years of relevant work experience
- 1–2 years of experience providing clinical support services
- At least 1 year of community outreach experience and knowledge of Louisiana community resources
- At least 1 year of experience working with clients with clinical needs, physical and/or behavioral health
- Proficiency with a personal computer, keyboard navigation, MS Office Suite, MS Teams, Outlook, Word, Excel, and electronic health record systems
- Flexibility to work outside core business hours Monday–Friday, 8am–5pm
- Ability to work two days weekly until 9pm local time
- High school diploma or G.E.D.
- Experience in community/outpatient settings preferred
- Preferred skills include Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions
- Experience serving Medicaid populations in urban or rural environments and familiarity with local formal and informal resource networks preferred
- Community Health Worker Certification or equivalent training and experience preferred
- Bilingual speaking skills in Spanish or French are a plus
