About the role
Key responsibilities & impact- Interact with Medicare members via digital and telephone channels
- Conduct inbound and outbound outreach regarding healthcare visits, screenings, vaccinations, and medication refills
- Capture barriers and collect data using motivational interviewing to help close gaps in care
- Provide professional, empathetic support and answer or triage member questions
- Advocate for Medicare members and coordinate care among health plans, doctors, and pharmacies
- Collect member-reported data in the CRM to support HEDIS medical-record documentation
- Educate members based on their needs, preferences, and understanding
- Serve as a single point of contact, resolving issues and escalating to leadership when needed
- Build trusting relationships and connect members with appropriate services
- Explain programs, Aetna tools, and healthcare resources
- Document and track member contacts, events, and outcomes in applicable systems
- Produce business communications, including letters and spreadsheets
- Handle multiple functions and products while meeting performance standards
- Identify issues requiring escalation and suggest resolutions
- Support individual, team, and business goals and initiatives
Requirements
What you’ll need- Windows-based application knowledge and ability to use standard corporate software packages and applications with a high degree of computer literacy
- Effective communication skills and experience speaking with medical providers and/or clinical staff
- Strong listening and interpersonal skills
- Strong analytical skills with accuracy and attention to detail
- Ability to de-escalate situations
- Empathy, curiosity, enthusiasm for learning, compassion, and listening skills
- Recent and related experience working with HEDIS screening measures and behavior change preferred
- Bilingual or multilingual candidates heavily preferred
- Bachelor’s degree or equivalent preferred
- Medical certification such as certified nursing assistant or medical assistant strongly recommended but not required
- Prior experience working directly with members/consumers, preferably in a call center setting
- Minimum 3 years of recent and related experience within healthcare, healthcare marketing, outreach and/or health education, health coaching, or disease management
- 3–5 years of experience in a healthcare setting with direct communication with various levels of management, including medical providers
- Telephonic coordination of care experience
- High School Diploma or G.E.D.
