About the role
Key responsibilities & impact- Provide in-home health evaluations in members’ private residences
- Conduct approximately 45-minute visits
- Review medications and medical history
- Perform physical evaluations
- Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
- Use an iPad and clinical workflow for each evaluation
- Visit people nationwide to connect them with appropriate care
- Provide Signify Health services, including Diagnostic & Preventive Services
- Conduct virtual visits as needed
- Travel locally and within licensed states as needed
- Build trusted relationships and help improve health outcomes
- Educate members and answer health questions without prescribing medication, ordering lab tests, or altering current treatment regimens
Requirements
What you’ll need- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Ability to work safely and comfortably in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants
- Flexibility to travel locally and within licensed states, as needed
- Ability to conduct virtual visits as needed
- Nurse Practitioner or Physician Assistant qualification
