About the role

Key responsibilities & impact

Provide in-home health evaluations in members’ private residences
Engage with members and build trusted relationships to improve health outcomes
Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations
Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry
Use an iPad and clinical workflow for each evaluation
Visit people nationwide and help connect them to appropriate care
Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening
Conduct virtual visits as needed
Travel locally and within licensed states as needed

Requirements

What you’ll need

Nurse Practitioner or Physician Assistant
Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Multi-state licenses (preferred)
Ability to travel locally and within licensed states, as needed
Ability to conduct virtual visits as needed
Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants
