About the role

Key responsibilities & impact

Provide in-home health evaluations in members’ private residences
Engage with members and build trusted relationships
Review medications and medical histories
Perform physical evaluations
Provide additional diagnostic and preventive services, including diabetic eye exams, spirometry, and bone density screening
Conduct virtual visits as needed
Travel locally and within licensed states as needed
Help identify connections to appropriate care and bridge gaps in care
Use an iPad and intuitive clinical workflow for each evaluation
Avoid prescribing medication, ordering lab tests, or altering current treatment regimens

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Board certification required
Multi-state licenses preferred
Nurse Practitioner or Physician Assistant qualification
Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants
Ability to travel locally and within licensed states as needed
Ability to conduct virtual visits as needed
Ability to use an iPad and clinical workflow
Ability to perform medication and medical history reviews, physical evaluations, and diagnostic tests as indicated
