About the role

Key responsibilities & impact

Provide In-Home Health Evaluations in members' homes
Engage with people and help bridge gaps in care
Travel primarily across a defined coverage area, as dictated by business needs
Conduct approximately 45-minute visits
Review medications and medical history
Perform physical evaluations
Conduct additional diagnostic tests when ordered by the person's health plan, including diabetic eye exams and spirometry
Use an iPad and clinical workflow for each evaluation
Conduct virtual visits as needed
Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening
Build trusted relationships and help improve health outcomes
Visit health plan members nationwide and connect them to appropriate care

Requirements

What you’ll need

Full-time Nurse Practitioner or Physician Assistant clinician
Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Able to obtain and maintain multi-state licenses in coverage area, as required
Able to cover all counties within state and flex to other areas of business need
Comfortable and able to safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
Able to travel locally and within licensed states
Willingness to travel up to 100% of the time, including possible overnight hotel stays, car rentals, and/or flights
Anticipated weekly hours: 40
