About the role
Key responsibilities & impact- Provide approximately 45-minute in-home health evaluations
- Engage one-on-one with health plan members and answer health questions
- Review medications and medical histories
- Perform physical evaluations
- Conduct additional diagnostic tests when ordered by the member’s health plan
- Build trusted relationships and help improve health outcomes
- Visit people in their homes to identify connections to appropriate care
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health services, including Diagnostic & Preventive Services products
Requirements
What you’ll need- Active, unrestricted license(s) in coverage area(s) Seattle Pierce, WA
- Board certification (required)
- Multi-state licenses (preferred)
- Nurse Practitioner or Physician Assistant qualification
- Ability to travel locally and within licensed states, as needed
- Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and other airborne allergens or irritants
- Ability to use an iPad and clinical workflow for evaluations
- Ability to provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening
