About the role

Key responsibilities & impact

Provide in-home health evaluations for health plan members
Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations
Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry
Use an iPad and clinical workflow for each evaluation
Build trusted relationships and help improve health outcomes
Visit people nationwide to connect them with appropriate care
Travel locally and within licensed states as needed
Conduct virtual visits as needed
Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screenings
Answer health questions and provide education without prescribing medication, ordering lab tests, or altering current treatment regimens

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Multi-state licenses (preferred)
Ability to travel locally and within licensed states as needed
Ability to conduct virtual visits as needed
Ability to safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants
Nurse Practitioner or Physician Assistant qualification
