About the role

Key responsibilities & impact

Provide part-time in-home health evaluations to health plan members
Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation
Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams and spirometry
Use an iPad and clinical workflow for each evaluation
Build trusted relationships and help improve health outcomes
Provide referrals to health plans and encourage participation of loved ones
Ensure primary care physicians are aware of patients’ health and quality of life
Conduct virtual visits as needed
Provide Signify Health diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening
Travel locally and within licensed states as needed

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Multi-state licenses preferred
Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
Ability to travel locally and within licensed states as needed
Ability to conduct virtual visits as needed
MD or DO qualification
