About the role

Key responsibilities & impact

Provide In-Home Health Evaluations in members’ private residences
Travel across a defined coverage area, potentially requiring overnight hotel stays, car rentals, and/or flights
Build trusted relationships and help improve health outcomes
Conduct approximately 45-minute visits
Review medications and medical histories
Perform physical evaluations
Perform additional diagnostic tests when ordered, including diabetic eye exams, spirometry, and bone density screening
Use an iPad and clinical workflow for each evaluation
Answer health questions and provide health education
Conduct virtual visits as needed
Provide Signify Health services, including Diagnostic & Preventive Services products
Cover all counties within the state and flex to other areas based on business needs

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Able to obtain and maintain multi-state licenses in coverage area, as required
Able to cover all counties within state and flex to other areas of business need
Comfortable and able to safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants
Nurse Practitioner or Physician Assistant qualification
Ability to travel locally and within licensed states
Ability to travel up to 100% of the time
Ability to conduct virtual visits as needed
Ability to use an iPad and clinical workflow
Ability to work 40 hours per week
