About the role
Key responsibilities & impact- Provide In-Home Health Evaluations in members’ homes
- Engage with people one-on-one and answer health questions
- Review medications and medical history
- Conduct physical evaluations
- Perform additional diagnostic tests, including diabetic eye exams, spirometry, and bone density screening, when ordered
- Use an iPad and clinical workflow for each evaluation
- Conduct virtual visits as needed
- Provide Signify Health services, including Diagnostic & Preventive Services
- Travel locally and within licensed states
- Build trusted relationships and help improve health outcomes
- Help connect people to appropriate care and identify care gaps
- Communicate findings and referrals to health plans and primary care physicians as appropriate
Requirements
What you’ll need- Active, unrestricted license(s) in coverage area(s)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- MD or DO qualification
- Ability to travel locally and within licensed states
- Ability to travel up to 100% of the time across a defined coverage area
- Ability to safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants
- Ability to conduct virtual visits as needed
- Ability to perform medication and medical history reviews, physical evaluations, and additional diagnostic tests when ordered
- 40 anticipated weekly hours
