About the role
Key responsibilities & impact- Provide In-Home Health Evaluations in members’ homes
- Travel across a defined coverage area, primarily up to 100% of the time
- Build trusted relationships and help improve health outcomes
- Conduct approximately 45-minute one-on-one visits
- Review medications and medical history
- Perform physical evaluations
- Perform ordered diagnostic tests such as diabetic eye exams and spirometry
- Use an iPad and clinical workflow for each evaluation
- Conduct virtual visits as needed
- Provide Signify Health services, including diagnostic and preventive services
- Visit people nationwide and connect them to appropriate care
- Refer members to their health plan and encourage loved-one participation when appropriate
Requirements
What you’ll need- Active, unrestricted license(s) in coverage area(s)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- MD or DO qualification
- Able to travel locally and within licensed states
- Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
- Willingness and ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals and/or flights
- Ability to conduct virtual visits as needed
