About the role

Key responsibilities & impact
  • Provide in-home health evaluations in members’ homes
  • Engage with people one-on-one and answer health questions
  • Review medications and medical history
  • Perform physical evaluations
  • Conduct additional diagnostic tests when ordered, including diabetic eye exams, spirometry and bone density screening
  • Use an iPad and clinical workflow for each evaluation
  • Conduct virtual visits as needed
  • Provide Signify Health services, including Diagnostic & Preventive Services
  • Travel locally and within licensed states
  • Build trusted relationships and help improve health outcomes
  • Identify care gaps and connect people to appropriate care

Requirements

What you’ll need
  • Active, unrestricted license(s) in coverage area(s)
  • Able to obtain and maintain multi-state licenses in coverage area, as required
  • MD or DO qualification
  • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
  • Flexible to travel locally and within licensed states
  • Willing and able to travel up to 100% of the time across a defined coverage area
  • Availability for overnight hotel stays, car rentals and/or flights as required
  • Ability to conduct virtual visits as needed
