About the role

Key responsibilities & impact

Provide in-home health evaluations in members’ residences
Engage one-on-one with people and answer health questions
Review medications and medical history
Conduct physical evaluations
Perform additional diagnostic tests when ordered, such as diabetic eye exams and spirometry
Use an iPad and clinical workflow for each evaluation
Build trusted relationships and help improve health outcomes
Travel locally and within licensed states, up to 100% of the time
Conduct virtual visits as needed
Provide Signify Health services, including Diagnostic & Preventive Services products
Visit people nationwide and connect them to appropriate care
May require overnight hotel stays, car rentals, and/or flights

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Able to obtain and maintain multi-state licenses in coverage area, as required
Able to cover all counties within California and flex to other areas of business need
Must be able to comfortably and safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants
Willing and able to travel locally and within licensed states up to 100% of the time
Multiple state licenses required as dictated by business needs
MD or DO qualification
