About the role

Key responsibilities & impact

Provide in-home health evaluations in members’ homes
Travel primarily across a defined coverage area, potentially requiring overnight hotel stays, car rentals and/or flights
Build trusted relationships and help improve health outcomes
Conduct approximately 45-minute visits
Review medications and medical history
Perform physical evaluations
Perform additional diagnostic tests when ordered, including diabetic eye exams and spirometry
Use an iPad and intuitive clinical workflow for each evaluation
Conduct virtual visits as needed
Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening
Travel locally and within licensed states
Meet health plan members in their private residences

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Able to obtain and maintain multi-state licenses in coverage area, as required
MD or DO qualification
Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
Willingness and ability to travel locally and within licensed states, up to 100% of the time
Ability to conduct virtual visits as needed
Anticipated weekly hours: 40
