Key responsibilities & impact

Provide In-Home Health Evaluations in members’ homes
Engage one-on-one with members and answer health questions
Review medications and medical histories
Perform physical evaluations
Conduct additional diagnostic and preventive tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening
Conduct virtual visits as needed
Provide all indicated Signify Health services, including Diagnostic & Preventive Services products
Travel locally and within licensed states across a defined coverage area
Visit members nationwide to help connect them to appropriate care
Build trusted relationships and help improve health outcomes

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Able to obtain and maintain multi-state licenses in coverage area, as required
MD or DO qualification
Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
Willingness and ability to travel locally and within licensed states, up to 100% of the time
Ability to conduct in-home and virtual health evaluations
Ability to use an iPad and clinical workflow
Ability to perform medication and medical history reviews, physical evaluations, and diagnostic tests as indicated
