About the role
Key responsibilities & impact- Provide In-Home Health Evaluations in members’ homes
- Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations
- Perform additional diagnostic tests when ordered, such as diabetic eye exams and spirometry
- Use an iPad and clinical workflow for each evaluation
- Conduct virtual visits as needed
- Provide Signify Health services, including Diagnostic & Preventive Services products
- Travel locally and within licensed states, potentially up to 100% of the time
- Build trusted relationships and help connect people to appropriate care
- Make referrals to health plans and communicate health and quality-of-life information to primary care physicians as appropriate
Requirements
What you’ll need- Full-time Physician role for MDs and DOs
- Active, unrestricted license(s) in coverage area(s)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
- Able to travel locally and within licensed states, including overnight hotel stays, car rentals and/or flights as needed
- Anticipated weekly hours: 40
