About the role
Key responsibilities & impact- Provide in-home health evaluations in members’ homes
- Engage with people one-on-one and help bridge gaps in care
- Review medications and medical history
- Perform physical evaluations
- Conduct additional diagnostic tests when ordered, including diabetic eye exams, spirometry, and bone density screening
- Conduct virtual visits as needed
- Provide Signify Health Diagnostic & Preventive Services
- Travel locally and within licensed states, potentially requiring hotel stays, car rentals, and flights
- Build trusted relationships and connect people to appropriate care
- Use an iPad and clinical workflow for each evaluation
- Avoid prescribing medicine, ordering lab tests, or altering current treatment regimens
Requirements
What you’ll need- Active, unrestricted license(s) in coverage area(s)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- MD or DO qualification
- Ability to travel locally and within licensed states
- Ability to travel up to 100% of the time across a defined coverage area
- Ability to perform duties safely in homes that may contain household pets, secondhand smoke, airborne allergens, or irritants
- Ability to conduct virtual visits as needed
- Ability to use an iPad and clinical workflow
- Ability to work 40 hours per week
