About the role

Key responsibilities & impact

Provide In-Home Health Evaluations in members’ homes
Engage with people one-on-one and help bridge gaps in care
Review medications and medical history
Perform physical evaluations
Conduct additional diagnostic and preventive tests when ordered, including diabetic eye exams, spirometry, and bone density screening
Use an iPad and clinical workflow for each evaluation
Conduct virtual visits as needed
Provide Signify Health services and Diagnostic & Preventive Services products
Travel locally and within licensed states, including possible overnight hotel stays, car rentals, and flights
Build trusted relationships and connect people to appropriate care

Requirements

What you’ll need

MD or DO physician qualification
Active, unrestricted license(s) in coverage area(s)
Able to obtain and maintain multi-state licenses in coverage area, as required
Ability to travel locally and within licensed states
Ability to travel up to 100% of the time across a defined coverage area
Ability to perform duties safely in homes that may contain household pets, secondhand smoke, airborne allergens or irritants
Ability to conduct in-home health evaluations, including medication and medical history review, physical evaluation, and diagnostic tests
Ability to use an iPad and clinical workflow
Ability to work 40 hours per week
