About the role

Key responsibilities & impact

Provide in-home health evaluations in members’ homes
Engage one-on-one with individuals and help bridge gaps in care
Review medications and medical history
Perform physical evaluations
Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry and bone density screening
Use an iPad and clinical workflow for each evaluation
Conduct virtual visits as needed
Provide Signify Health services, including Diagnostic & Preventive Services products
Travel locally and within licensed states
Build trusted relationships and connect people to appropriate care
Visit people nationwide across a defined coverage area

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Able to obtain and maintain multi-state licenses in coverage area, as required
MD or DO qualification
Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
Flexibility to travel locally and within licensed states
Ability to travel up to 100% of the time across a defined coverage area
Ability to accommodate overnight hotel stays, car rentals and/or flights as required
Ability to conduct virtual visits as needed
