About the role

Key responsibilities & impact

Evaluate members using care management assessments and information/data review
Recommend approaches to resolve care needs and maintain optimal health and well-being
Identify high-risk factors and service needs affecting member outcomes
Implement proactive support interventions and Wellness care plan activities
Monitor member care needs
Consult with case managers, supervisors, Medical Directors and other health programs to overcome barriers
Present cases at case conferences for multidisciplinary review
Identify and escalate quality-of-care issues through established channels
Negotiate appropriate options and services to meet members’ benefits and healthcare needs
Use motivational interviewing to promote member engagement and lifestyle/behavior changes
Provide coaching, information and support for independent medical and healthy lifestyle choices
Help members participate knowledgeably in healthcare decision-making with providers
Apply case management and quality management processes in compliance with regulatory, accreditation and company guidelines
Travel to members’ homes, schools, hospitals, doctors’ offices and DCBS offices for face-to-face member contact
Travel to the Louisville office for meetings and training

Requirements

What you’ll need

Minimum 6 months experience in Foster Care and juvenile justice systems, Adoption Assistance, the delivery of Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention services or evidence-based practices applicable to the Kentucky SKY populations
Minimum 2 years of experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling
Minimum 1 year of experience in MS Office Suite applications, including Word and Excel
Willing and able to travel 80-90% of the time to meet members face to face in the assigned region
Must reside in Jefferson County, Kentucky
Minimum of a Bachelor’s Master’s degree or a non-licensed master level clinician in behavioral health or a relevant human services field of study, including social work, psychology, marriage and family therapy, or counseling
Flexibility to work beyond core business hours of Monday-Friday, 8am-5pm EST
