About the role
Key responsibilities & impact- Support primary care providers with clinical documentation so they can focus on patient care
- Huddle with providers each morning to plan the day's visits
- Join providers in exam rooms to observe and document patient visits
- Document patient encounters in structured notes, including history of present illness, assessment, plan, and physical exam
- Assign appropriate CPT and ICD-10 codes
- Prepare After Visit Summaries
- Consult with providers to ensure accurate and specific documentation
- Request and review medical records
- Use Oak Street Health population health tools to support clinical documentation improvement
- Prepare for and support Daily Huddles and Clinical Documentation Reviews
- Consult with providers on clinical documentation opportunities
- Place orders and referrals
- Address tasks and support the care team with additional clinical documentation responsibilities
- Perform other duties as assigned
Requirements
What you’ll need- Knowledge of medical terminology and common medications, either from a pre-medical degree or prior clinical experience
- Advanced listening and communication skills
- Strong computer literacy and ability to learn new technical workflows
- Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities served, where indicated
- Ability to adapt to new workflows and quickly learn new concepts and skills
- Ability and willingness to take direction and be a member of a patient-care team, including adapting to the provider's working style
- Ability to be a self-starter within role scope
- Excellent job attendance and ability to work in-person in clinics
- Ability to commit to at least 1 year in role; 2+ years ideal
- Ability to work approximately 40–45 hours per week during clinic hours
- Compliance with hospital and Oak Street Health policies, including HIPAA
- US work authorization
- Prior clinical experience, including shadowing and/or volunteering, strongly preferred
- Prior scribe or transcription experience preferred but not required
- Ability to type 70+ words per minute strongly preferred
