About the role

Key responsibilities & impact

Provide approximately 45-minute In-Home Health Evaluations in members’ homes
Engage one-on-one with members and answer health questions
Review medications and medical history
Perform physical evaluations
Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
Build trusted relationships and help improve health outcomes
Visit health plan members nationwide and connect them with appropriate care
Travel locally and within licensed states as needed
Conduct virtual visits as needed
Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening
Use an iPad and clinical workflow for each evaluation
Do not prescribe medicine, order lab tests, or alter current treatment regimens

Requirements

What you’ll need

Nurse Practitioner or Physician Assistant
Active, unrestricted license(s) in coverage area(s)
Board certification required
Multi-state licenses preferred
Ability to travel locally and within licensed states as needed
Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants
Ability to use an iPad and clinical workflow for evaluations
Ability to perform duties safely and comfortably in diverse home environments
