About the role

Key responsibilities & impact

Provide approximately 45-minute in-home health evaluations
Engage with health plan members in their homes and build trusted relationships
Review medications and medical history
Perform physical evaluations
Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
Use an iPad and clinical workflow for each evaluation
Visit people nationwide to connect them with appropriate care
Travel locally and within licensed states as needed
Conduct virtual visits as needed
Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening
Work primarily in private residences of health plan members

Requirements

What you’ll need

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Multi-state licenses (preferred)
Nurse Practitioner or Physician Assistant qualification
Ability to travel locally and within licensed states, as needed
Ability to conduct visits in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
Ability to use an iPad and intuitive clinical workflow
Anticipated weekly hours: 12
